Case Manager (Human Services Case Manager)
Bilingual Preferred (Spanish & English)
Salary Range: $5,193 - $6,904

Summary of Duties- Assess Oregonians applying for, or currently receiving, long‑term care services and develop, coordinate, and arrange a variety of services to enable older adults and people with disabilities to receive continuity of care.
- Provide choice counseling and establish, develop, and implement case management plans for in‑home, community‑based, and nursing home care.
- Contact Oregonians or collateral contacts monthly—by in‑person, phone, or electronic methods—to set up and monitor service plans and safety needs.
- Coordinate the provision of long‑term care services in accordance with rules, regulations, and individual case plans.
- Assist with eligibility determinations for healthcare, social services, and protective service needs, using the ONE System and the Case Assessment and Planning System (CA/PS).
- Collaborate with Adult Protective Services, AFH Licensors, and other agencies to ensure safety, protection, and compliance with mandatory reporting guidelines.
- Ensure service delivery is culturally competent, trauma‑informed, and available in multiple languages and formats; provide bilingual services where needed.
- Process applications, inquiries, and data according to policy time frames; maintain accurate documentation and communicate eligibility decisions.
- Three years of human services related experience (e.g., assisting individuals and groups with issues such as economic disadvantage, employment, abuse and neglect, substance abuse, aging, disabilities, housing).
- Or a bachelor’s degree in a Behavioral Science, Social Science, or a closely related field.
- Or an associate degree in a related field plus two years of human services related experience.
- Or an equivalent combination of education, experience, and training.
- Experience with case assessment and planning tools and the ONE System is strongly preferred.
- Experience coordinating services and support, assessing needs, developing service plans, monitoring progress, and adjusting services to promote positive outcomes.
- Strong interpretation and application of laws, regulations, policies, and procedures to make consistent, accurate, and well‑supported decisions.
- High‑quality, timely documentation and clear communication of actions taken.
- Effective interview and information‑gathering skills to assess needs and determine appropriate next steps.
- Ability to work independently and collaboratively in a fast‑paced environment with evolving policies and procedures.

- Requirements for a valid driver’s license and acceptable driving record to use state vehicles.
- Exposure to potentially stressful situations and need for quick decision‑making to ensure safety.
- Criminal history and background check required for finalists; adverse results may lead to disqualification.
- Mandatory completion of the U.S. Department of Homeland Security I‑9 form and E‑Verify within three days of hire.
