The Oregon Department of Human Services is seeking a Case Manager to assess applicants for long‑term care services, coordinate supports, and develop individualized care plans for older adults and people with disabilities. Bilingual preferred (Spanish/English).
In‑office with occasional remote work possible with manager approval; full‑time schedule, Monday–Friday. Experience in case management and familiarity with the ONE System are preferred.
