OHI Inc. is seeking a Full-Time Community Case Manager—Remote, based in the Bangor, Maine area. The role supports adults with disabilities to live self-directed lives and participate in their communities.
You will identify needs, develop plans of care, coordinate services, monitor progress, and maintain clear documentation. A bachelor’s degree in a related field and at least one year of experience are required; CPR/First Aid certification and CDS readiness are expected.
