Summary of DutiesAs a Case Manager, you will assess Oregonians applying for or receiving long‑term care services and develop, coordinate, and arrange a variety of services to ensure continuity of care for older adults and people with disabilities.
- Provide case management by coordinating the provision of long‑term care services to Oregonians according to rules, regulations, and individual case plans.
- Contact Oregonians or collateral contacts monthly to set up and monitor service plans and safety needs via in person, phone, or electronic contact.
- Ensure service delivery is culturally competent and trauma‑informed, with printed materials available in multiple languages and formats.
- Collaborate with the eligibility team and use the ONE System to review and update Long‑Term Care Services.
- Interview Oregonians to determine eligibility for healthcare, social services, and protective services; develop detailed, individualized, person‑centered case plans.
- Conduct interviews in person/on‑site using the Case Assessment and Planning System (CA/PS) assessment tool on a state‑issued laptop.
- Process applications, inquiries, and data according to policy time frames, ensuring appropriate documentation and communication of eligibility decisions.
- Work collaboratively with Adult Protective Services, AFH Licensors, and other agencies to ensure safety and protection of Oregonians, following mandatory reporting guidelines.
- Three years of human services experience (e.g., assisting economically disadvantaged individuals, abuse or neglect, substance abuse, aging, disabilities, health, or housing).
- Or a bachelor’s degree in Behavioral Science, Social Science, or a related field.
- Or an associate degree in a related field plus two years of human services experience.
- Or an equivalent combination of experience, education, and training.
- Experience coordinating services and support for individuals, assessing needs, developing service plans, monitoring progress, and adjusting services to promote positive outcomes as a case manager.
- Experience interpreting and applying laws, regulations, policies, or procedures to make consistent, accurate, and well‑supported decisions.
- Experience preparing clear, accurate, and timely documentation that supports decisions and complies with organizational or legal requirements.
- Experience conducting interviews or information‑gathering conversations to assess needs, evaluate available information, and determine appropriate next steps using sound judgment.
- Work schedule:
8:00 a.m. – 5:00 p.m., Monday – Friday Pacific Standard Time; occasional overtime may be available. - Work largely in an office and community setting; some opportunities for remote work subject to manager approval.
- Travel may be required for business needs or training classes.
- Must possess a valid driver’s license and acceptable driving record for state vehicle use.
