Float Care Coordinator, LSW; MSHO

Position Overview

Float Care Coordinator responsible for managing Minnesota Senior Health Options (MSHO/MSC+) and potentially SNBC members in community, skilled nursing, and assisted living settings. The role is performed from home with regular local travel.

• Full‑time, day shift: 8 am–5 pm Monday–Thursday, 8 am–3 pm Friday.

• Approximately 50–70 % travel within the seven‑county metro area.
• Approximately 30–50 % remote work.

• $65,000 – $75,000, commensurate with experience.

• Manage member caseloads within assigned geographic area.
• Coordinate face‑to‑face visits.
• Manage the Elderly Waiver.
• Conduct annual PCA assessments and develop customized living tools for Assisted Living members.
• Review health needs, set goals, and develop individualized care plans.
• Connect members with community and state resources and services.
• Complete required documentation.
• Collaborate with medical teams to meet quality measures and utilize utilization management tools to achieve value‑based goals.
• Support members during transitions of care, ensuring safe discharge and follow‑up plans.

• Current MN Licensed Social Worker (new graduates encouraged) or MN Registered Nurse license.
• At least one year experience in geriatric or certified disabled population case management/care coordination in settings such as Home Care, Group Homes, Nursing Homes, TCU, or Assisted Living preferred.
• Valid driver’s license.
