Moms In Motion is seeking a Service Facilitator (SF) Case Manager to connect with families across Virginia, helping them access CCC+, CL & FIS, and EPSDT waiver programs. You’ll visit clients in their homes, complete plans of care, and support families to live safer, healthier, and more independent lives.
This role is remote, with local travel. A minimum of 2 years’ experience is required; no degree is needed. Earn $40,000–$55,000 annually plus benefits, paid training, and ongoing development.
