Schedule: Monday to Thursday, 8am to 6pm (flexibility available within this schedule)

PositionThis role is part of an interdisciplinary team that includes nurses, behavior therapists, and public benefits administrators. The professional plans, monitors, and coordinates the operations of health and habilitation programs for consumers in community settings, ensuring a high quality of life.
Responsibilities- Training community providers and staff, care planning, and problem solving
- Managing a caseload of consumers with intellectual and developmental disabilities living in community homes, apartments, or family homes
- Overseeing all aspects of the consumer's care to ensure each experiences a high quality of life based on their abilities
- Carrying out monitoring and quality assurance responsibilities in accordance with the organization’s policies, procedures, personnel manual, and applicable laws and regulations
- Associate’s Degree or equivalent
- At least 2 years of related experience and/or training
- Bachelor’s Degree or equivalent
- 2 to 5 years of related experience and/or training
- Equivalent combination of education and experience may be considered
- Bilingual (English and Spanish)
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations; write reports, business correspondence, and procedure manuals; effectively present information and respond to questions from managers, consumers, guardians, family members, and the general public.
Reasoning AbilityAbility to solve practical and complex problems, critically reason through variables in situations with limited standardization, interpret written, oral, diagrammatic, or schedule instructions, and prioritize work to manage time effectively.
Work EnvironmentThe work environment varies between remote work, office, and community locations including host homes and hospitals. A personal vehicle is required for transportation to monitor and coordinate care. The home office is located in Aurora, CO.
Physical DemandsRegularly required to reach with hands and arms, talk or hear, and drive. Occasionally lift and/or move up to 50 pounds. Specific visual abilities include close and distance vision, color vision, depth perception, and ability to adjust focus.
