Mercy First in Brooklyn supports foster youth through the Bridges to the Future program, helping youth access quality schools and resources. You will advocate, coordinate referrals, and collaborate with families, schools and agency staff to ensure educational success.
This role focuses on creating individualized education plans, coordinating referrals for tutoring, and tracking progress with stakeholders to promote positive outcomes for youth in care.
