Direct Jobs is seeking a Social Worker (HBPC) to support the TVHS Home Based Primary Care program in Hopkinsville, KY. The role focuses on removing barriers to care and assisting Veterans and families with psychosocial, emotional, and economic needs within a multidisciplinary team.
The candidate will provide psychosocial assessments, develop care plans, and coordinate referrals to VA programs and community resources, including telehealth as appropriate.
