Moms In Motion is seeking a Service Facilitator (SF) Case Manager who will help families access Virginia’s Medicaid Waiver programs (CCC+, CL & FIS, EPSDT) and support independent, healthy living. You’ll visit clients in their homes and coordinate plans of care with compassion and organization.
Must live in VA with a valid driver’s license and reliable vehicle; no degree required. You’ll also enjoy paid training, benefits, and flexible, laptop-based remote work when not visiting clients.
