Roots to Grow is seeking highly motivated, tech-savvy, and detail-oriented Screener/Navigators to join our growing team serving communities throughout New York City.
This is a
hybrid position
with
most work performed remotely from home
, along with occasional in-office meetings, trainings, community events, and field-based activities as needed. We are looking for individuals who are comfortable working independently in a fast-paced environment while leveraging technology to efficiently manage their workload.
This role is ideal for someone who enjoys helping others while also being highly organized, adaptable, and confident using computers and multiple software platforms throughout the day.
What You'll Do- Conduct comprehensive social care screenings with clients by phone and through virtual platforms.
- Assess client needs and connect individuals with appropriate community resources and services.
- Maintain accurate, detailed, and timely electronic documentation.
- Manage multiple cases while meeting productivity and quality expectations.
- Navigate between multiple software platforms, databases, spreadsheets, and web-based systems throughout the workday.
- Communicate professionally with clients, healthcare partners, and community organizations.
- Participate in virtual meetings, occasional in‑person trainings, office meetings, and community outreach events.
- Work independently while remaining engaged with supervisors and team members in a remote environment.
This position is heavily technology-driven. Successful candidates must be comfortable learning new software quickly and working in multiple systems simultaneously.
Candidates should have experience with:
- Microsoft Excel (sorting, filtering, spreadsheets, and data entry)
- Microsoft Word
- Google Workspace (Gmail, Google Drive, Google Sheets, Google Docs, Google Meet)
- Web‑based databases and case management systems
- Accurate keyboarding and data entry
Strong computer proficiency is required and will be evaluated throughout the hiring process. Applicants should be confident using technology independently and be able to quickly learn new systems and workflows.

- High preference given to bilingual and multilingual candidates.
- Previous experience in healthcare, social services, customer service, navigation, care coordination, or case management is preferred but not required.
- Primarily work from home with a hybrid schedule.
