Moms In Motion, a Virginia-based service facilitator provider, seeks a Service Facilitator (SF) Case Manager to connect families with Medicaid Waiver programs and help them live safe, healthy, independent lives.
We welcome candidates with at least 2 years of relevant experience and no degree requirement. Local VA travel, a valid driver’s license, and strong communication and tech skills are essential to coordinate plans of care and remote work.
