A well-established, privately funded nonprofit dedicated to helping older adults age safely and independently is looking for a Social Worker / Geriatric Care Manager to join its care management team. This is a direct-hire opportunity with a small, tight-knit group of social workers and a genuinely supportive culture - turnover here is rare.
The RoleYou’ll carry a manageable caseload of older adults living in the community, with the majority of cases based in Bergen County, NJ
, plus occasional cases in South Jersey and New York.
- Provide comprehensive care management and ongoing support to community-dwelling seniors
- Conduct in-home visits and assessments (minimum one in-person visit per client, per month)
- Advocate for clients navigating healthcare, housing, financial, and social service systems
- Connect clients to benefits, entitlements, and community resources
- Develop individualized service plans and monitor progress
- Collaborate with families, healthcare providers, and support networks
- Participate in weekly team meetings and case conferences
Hybrid. 1 day in office office for team meetings. All other days are a mix of remote work and field-based home visits, with flexible scheduling outside of the in-office day.
What We're Looking For- LMSW or LCSW preferred
- strong MSW/Masters Level candidates with relevant experience will be considered - Experience with older adults, care management, case management, entitlements, or related human services
- Working knowledge of Medicare, Medicaid, Social Security, senior housing, and community-based services
- Excellent interpersonal, communication, and advocacy skills
- Strong organization, documentation, and time-management abilities
- Comfortable working independently in the field while staying connected to a collaborative team
- Proficiency with Microsoft Office and electronic documentation systems
- Valid driver's license and reliable vehicle required - this role involves regular field work
