Bluestone Physician Services is seeking a Care Coordinator who will support Minnesota Senior Health Options (MSHO/MSC+) members in the community and assisted living facilities. The role is primarily work-from-home with 50-70% local travel within Blaine area and surrounding MN communities.
You will coordinate visits, assess needs, develop care plans, and connect members with community resources while collaborating with multidisciplinary care teams.
