ACC Community Connections seeks a Forensic Case Manager to deliver community-based case management services (FORENSIC focus) for clients in Fond du Lac and surrounding areas. Travel to client homes and community settings is required, with collaboration across probation, law enforcement, courts, DHS, and providers to support recovery and safety.
