Job Description

Under the supervision of the Site Supervisor, the Criminal Justice Liaison is responsible for outreach to members who are involved in the criminal justice system or are currently in jail. The Liaison will coordinate with the Pima County and State courts, the member, and the treatment Team to ensure timely follow through on community re-entry, appointment adherence and additional support during follow up appointments.

The liaison will coordinate and monitor successful compliance with mental and physical health recommendations.

Job Responsibilities
  • Successfully coordinate with criminal justice resources.
  • Coordinate intensive outreach and peer support services to members in order to avoid gaps in services and to provide continuity of care (including but not limited to: diversion, educational activities, med cueing, environmental assessments, transportation, referrals, ART meetings, etc.).
  • Coordinate with Treatment Team, criminal justice resources, court system, probation/parole officers, mental health court, etc.
  • Provide restoration of competency services for the Superior Court.
  • Facilitate psycho educational groups for members involved in the criminal justice system to decrease recidivism.
  • Complete documentation for behavioral health record in accordance with agency and funding source requirements.
  • Participate in the assessment, evaluation and planning of patient care.
  • Provide case management, health promotions, skills training, and assessment when engaging with members involved in criminal justice services.
  • Provide community and home-based services to enhance and achieve treatment goals.
  • Maintain a minimum number of documented client contact hours per week.
  • Maintain appropriate documentation (charting) and documents accurately and timely in accordance with agency and funding source requirements.
  • Coordinate transportation of members to and from appointments when appropriate.
  • Provide outreach and follow-up on crisis contacts and missed appointments.
  • Provide member advocacy and education as needed, to ensure delivery of required services.
  • Perform other related duties in accordance with agency growth and changes.
  • Preserve the human dignity and the confidentiality of each patient.
  • Perform other duties as assigned or necessary as it relates to the general nature of the position.
Qualifications
  • Education and Experience – Bachelor's degree in a behavioral Health related field with one year behavioral health experience, OR, BHT level with two years behavioral health experience specific to program position.
  • Regulatory
    • Minimum 21 years of age.
    • Arizona Level One Fingerprint Clearance Card (must possess upon hire and maintain throughout employment).
    • CPR, First Aid & AED certification, if required (must possess upon hire and maintain throughout employment).
    • Valid Arizona Driver's License, 39-month Motor Vehicle Report, proof of vehicle registration and liability insurance that meet company insurance requirements, if required.
    • Negative TB test result, if required (Employer provides)
