Job Overview – Social Worker / Geriatric Care Manager
Compensation: $65,000 – $75,000/year

TAG Med Staffing is hiring a Social Worker / Geriatric Care Manager in Bergen County, NJ for our client, supporting older adults through personalized care management, advocacy, and community-based services. This hybrid role combines remote work, home visits, and weekly in-office collaboration, allowing social workers to build meaningful relationships while helping seniors maintain their independence and quality of life. The ideal candidate is compassionate, organized, and passionate about serving the aging population.
Responsibilitiesas the Social Worker / Geriatric Care Manager:
- Care Management: Conduct comprehensive assessments, develop individualized care plans, and provide ongoing case management for older adults living independently in the community.
- Client Advocacy: Assist clients in accessing healthcare, housing, financial assistance, public benefits, and community resources while advocating for their long-term well-being.
- Home Visits: Conduct regular in-home visits throughout the assigned territory, monitor client progress, and identify changing care needs.
- Care Coordination: Collaborate with family members, healthcare providers, and community organizations to coordinate services and ensure continuity of care.
- Documentation & Compliance: Maintain accurate case records, complete timely documentation, and participate in case conferences and team meetings.
- Program Support: Contribute to quality improvement initiatives, support program development, and maintain strong relationships with community partners and referral sources.
- Education: Master's degree in Social Work (MSW) is required.
- Licensure: LMSW or LCSW is preferred; qualified MSW candidates with relevant experience will also be considered.
- Experience: Experience in geriatric care management, case management, social work, aging services, or a related human services field is required.
- Industry Knowledge: Knowledge of Medicare, Medicaid, Social Security, senior housing, community resources, and care coordination for older adults is preferred.
- Technical
Skills:
Proficiency with Microsoft Office and electronic documentation or case management systems is required. - Skills & Attributes: Strong interpersonal, communication, advocacy, organizational, and time-management skills with the ability to work independently, build trusting client relationships, and collaborate effectively within a close-knit team. A valid driver's license and reliable vehicle are required, with mileage and toll reimbursement provided for field travel.
