Care Source is seeking an Onboarding Specialist in Massachusetts to serve as the primary contact for members transitioning to a Care Source health plan. You will conduct telephonic outreach, identify ongoing services and new requests, educate members on plan offerings, and coordinate with providers to ensure smooth transition and continued care.
You will schedule initial nurse intake visits and use motivational interviewing to build trust and encourage participation.
