We are seeking a highly motivated, innovative, and enthusiastic staff member to join our Benefits and Community Support (BCS) Division as a Case Manager.
Case Managers provide professional/technical case management and direct service and assistance to clients within the Employment First and/or Colorado Works Program through the Department of Human Services. This position assists clients in identifying barriers to entering and maintaining employment, and provides assistance and referrals to enable clients to overcome barriers with the goal of entering and maintaining employment.
The typical schedule for this position is Monday-Friday, 8 AM - 5 PM. This position is a hybrid position and will require at least one day/week in the office. You will be required to have adequate internet access and space for remote work. Larimer County provides a one-time remote work stipend to assist with equipment and office supply needs for remote work spaces.

What you'll be doing:
- Providing individualized case management services to assist clients with the ultimate goal of employment.
- Conducting assessments to identify individual client needs and goals.
- Determining program eligibility and administering services within local, state, or federal guidelines.
- Providing crisis intervention and coaching to assist clients in resolving financial, medical, or other barriers to obtaining and maintaining employment.
- Developing goal plans, making referrals, and coordinating services to meet individual client needs.
- Completing data entry to document goal plans, client progress, and services provided.
- Acting as an advocate for the client; coordinates with collateral agencies, families, or service providers; provides follow-up case management to assist clients in resolving problems in receiving quality care and/or services.
- Responding to requests for information and acting as a technical and referral source for clients and the public regarding county and community programs.

What We're Looking For- Have the ability to think critically and problem-solve and be aware of principles, practices, and objectives as related to human behavior and case management in a human services program.
- Have knowledge of the scope and application of laws and regulations pertaining to the assigned and related human services programs.
- Possess practices and techniques of counseling and interviewing.
- Provide available community resources including employment, medical, social, educational, and financial.
- Be responsible for record-keeping and reporting procedures.
- Maintain sensitive and confidential information.
- Communicate clearly and concisely, both verbally and in writing.
All candidates will be screened for the following required minimum qualifications. Any candidate who does not meet all of the qualifications listed as required will be eliminated from the process.
Depending on qualifications this position may be hired as a Case Manager I or II.
Case Manager I $24.1868/hr - $29.0241/hr
- Equivalent to a Bachelor's Degree from an accredited college or university required. Combination of education and/or experience may be substituted on a year for year basis.
- Possession of a valid Driver's License by date of hire required.
- Complete training and/or certification as mandated by the assigned program required.
