United Healthcare seeks a LTSS case manager to support intake, screening, referral, assessment and ongoing case management for members in Colorado. You will document contacts, develop and update care plans, and monitor health and welfare outcomes in our prescribed systems.
Role requires travel up to 50% within the territory, confidentiality adherence, and collaboration in quarterly and annual assessments. A Bachelor's in human behavioral sciences or substantial LTSS experience is required.
