Schedule:

Primarily Monday–Friday, 40 hours per week, (one work-from-home day after the probationary period) with flexibility to support occasional evenings, weekends, holidays, and on-call responsibilities.

Job Overview

Wellspring is seeking a dedicated and compassionate Community Living and Supports Resource Coordinator to coordinate and support all aspects of service access, implementation, and quality oversight for individuals receiving Home and Community-Based Services (HCBS). This position serves as a key resource for individuals receiving Developmental Disability (DD) Waiver, Supported Living Services (SLS) Waiver, Community First Choice (CFC) services, and transportation supports, ensuring services are coordinated, person-centered, compliant, and aligned with each individual's goals, preferences, and desired outcomes.

The Community Living and Supports Resource Coordinator will collaborate with individuals, families, guardians, Case Management Agencies (CMAs), service providers, Wellspring leadership, and community partners to promote independence, meaningful community participation, quality outcomes, and compliance with applicable Medicaid waiver requirements.

Essential Responsibilities
  • Coordinate the intake and onboarding process for individuals accessing HCBS services, including DD Waiver, SLS Waiver, CFC, and transportation services.
  • Facilitate authentic person-centered planning meetings and support the development of individualized goals and objectives in collaboration with the person receiving services.
  • Ensure individual preferences, strengths, needs, and desired outcomes guide service planning and implementation.
  • Develop and maintain collaborative relationships with individuals, families, guardians, Case Management Agencies (CMAs), Support Coordinators, service providers, and community partners.
  • Assist with the development, implementation, and monitoring of individualized service plans to ensure supports are delivered according to identified goals, objectives, and authorized services.
  • Conduct initial intake assessments, including reviews of independent living skills, health and safety needs, emergency preparedness, and support requirements in collaboration with the Program Manager, Nurse, and other members of the interdisciplinary team.
  • Conduct quarterly monitoring visits and service reviews to evaluate service implementation, ensure individual needs are being met, assess health and safety considerations, and verify compliance with HCBS requirements and Wellspring standards.
  • Monitor documentation, service records, and support practices to ensure alignment with Medicaid waiver requirements, person-centered planning principles, and organizational policies and procedures.
  • Identify areas for improvement, coordinate follow-up actions when needed, and collaborate with internal teams and external partners to promote continuous quality improvement.
  • Ensure timely communication and follow-up with individuals, families, CMAs, and service providers regarding service changes, concerns, and identified needs.
  • Maintain accurate and timely documentation within organizational databases, electronic charting systems, and required state systems.
  • Support compliance activities, including internal reviews, quality assurance monitoring, regulatory readiness, and corrective action planning when necessary.
  • Assist individuals in identifying and accessing community resources that promote independence, inclusion, self-direction, and meaningful community participation.
  • Maintain knowledge of Colorado Medicaid waiver requirements, HCBS Settings Rule expectations, and applicable state and federal regulations.
  • Ensure services are delivered in a manner that promotes individual rights, dignity, choice, autonomy, and community inclusion.
  • Complete required quarterly monitoring activities, documentation reviews, and quality assurance checks to support ongoing compliance and effective service delivery.
