The PACE Senior Care Manager — Prevention, Assessment, Connections, Empowerment — plays a key role in helping families stay together, strengthening kinship and natural supports, supporting permanency, and empowering youth and families through voluntary and court-involved services.

Using family-centered, trauma-informed, and strength-based practices, this position leads family finding and engagement efforts, coordinates support networks, assists with placement stabilization and reunification planning, and works closely with families, staff, providers, and legal partners to create safe, lasting solutions.

What You'll Do

• Engage youth and families through prevention, placement, permanency, and aftercare services.
• Identify relatives, kinship connections, and natural supports to strengthen family networks.
• Support placement prevention, stabilization, reunification, transition planning, and long-term permanency.
• Provide voluntary programming and family-centered support.
• Lead family finding using ecomaps, genograms, discovery tools, interviews, and approved databases.
• Coordinate family-centered meetings, permanency consultations, and case staffings.
• Consult with Initial and Ongoing Care Managers on family finding, kinship support, placement planning, and aftercare needs.
• Maintain timely documentation, required forms, correspondence, and outcome tracking in eWiSACWIS and other systems.

Work Schedule

Primarily Monday through Friday, 8:00 a.m. to 4:30 p.m., with some flexibility required to meet the needs of families. Remote work up to 50% of the time to be determined based on program eligibility.

Minimum Training & Experience Requirements 1. A bachelor's degree from a recognized college or university in sociology, social work, psychology, nursing, criminal justice, or a closely related field.
2. Two years of responsible work experience in social work, counseling, nursing, criminal justice, or a closely related field.
3. A master's degree in social work, nursing, psychology, criminal justice, or a closely related field may substitute for one year of the work experience.
4. Social work certification may substitute for one year of work experience.
5. Valid driver's license.
