Presbyterian Children’s Homes and Services (PCHAS) is seeking a Foster Care Case Manager to ensure safety and services for children in foster care. You will coordinate individualized care plans, facilitate Family Support Team meetings, and link families with appropriate community resources.
You will maintain records in state databases, visit families regularly, participate in court proceedings, and collaborate with schools, providers, and agency partners to promote permanency and well-being.
