The Coordinating Center in Maryland seeks a Case Manager 2 for the Medical Day Care Services Waiver program. This full-time role provides ongoing professional case management and advocacy for applicants/participants while adhering to COMAR regulations and MDH guidelines.
You will develop POS for clients, coordinate care via telephonic outreach and on-site visits, assess needs, and connect clients to resources. Strong communication, organization, and collaboration with agencies are essential.
