Humana is seeking a Field Service Coordinator (Care Coach
1) to assess member needs, guide families to appropriate resources, and support LTSS services. You will meet members in person or remotely, evaluate barriers, and connect them with services to promote well-being and better health outcomes.
Responsibilities include planning and modifying care plans, coordinating with the care team, and helping members navigate LTSS so they can access needed support and maintain Medicaid eligibility.
