JASA in New York invites a Case Manager to support older adults by connecting them with benefits, services, and programs. You will provide information and referral and case assistance in person and remotely, helping participants navigate resources and advocate for their needs.
Under supervision of the Program Director, you assess needs, assist with applications, advocate for services, and coordinate with community partners.
