Moms In Motion is seeking Service Facilitators to connect with families across Virginia, helping them navigate Medicaid Waiver programs and ensuring safe, healthy, independent living.
As a Service Facilitator, you will visit clients in their homes, guide families through CCC+, CL & FIS, and EPSDT waivers, and develop plans of care and assessments that actually make a difference. Remote work from a laptop is possible.
