At the Lackawanna County Department of Aging, we are hiring an Aging Care Manager
1. This role is trained to conduct Protective Service assessments and investigations, develop effective care plans, and collaborate across teams to ensure consumer needs are met. Our goal is to improve the quality of life for older adults by providing them with access to a range of community and home-based services. of Work
Aging Care Managers are trained to assess the needs of older adults; develop and implement care plans; and provide advocacy, ongoing care management, and key services to protect consumers from abuse, neglect, exploitation, and abandonment. As an Aging Care Manager 1, you will learn and perform the following duties:
- Care Management: Assess consumer needs, develop and implement care plans to stabilize consumers in their living environment or arrange for a better daily situation
- Protective Service Investigations: Conduct interviews, obtain relevant documents, and use standardized assessment forms to analyze facts, recognize when a consumer is at risk, and initiate appropriate steps to provide protection
- Home Visits: Travel to each consumer's home to verify their safety and welfare, provide connections to available resources, and measure progress toward goals
- Collaboration: Work with consumers, their families, other providers, and law enforcement to complete investigations and support consumer well-being
- Documentation: Record case information and activities accurately, in a timely manner, and according to agency policy and procedures
Work Schedule and Additional Information:
- Full-time employment
- Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 30-minute lunch.
- You must be able to travel to areas that may or may not be accessible by public transportation.
- Telework: You will not have the option to telework in this position.
- Salary: In some cases, the starting salary may be non-negotiable.

Qualifications
Minimum Experience and Training Requirements:
- Three years as an Aging Case Aide 2; or
- A bachelor's degree which includes or is supplemented by twelve (12) credits in social sciences, behavioral sciences, human services, or a closely related field; or
- An equivalent combination of experience and training.
- Applicants will be considered to have met the educational requirements once they are within three months of graduating with a qualifying degree/credits.

- Hiring preference for this vacancy may be given to candidates who live within Lackawanna County. If no eligible candidates who live within Lackawanna County apply for this position, candidates who reside in other counties may be considered.
- You must be able to perform essential job functions.

- This position falls under the provisions of the Older Adult Protective Services Act. Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
- This position falls under the provisions of the Child Protective Services Law. Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
