Lutheran Social Services of Wisconsin and Upper Michigan (LSS) is seeking a Care Coordinator for the CLTS Waiver Program in Milwaukee County. You will be the primary contact for families, conducting assessments, planning services, and coordinating supports to keep children safe and thriving at home and in the community.
Work involves collaboration with families, providers, and supervisors, with travel required and a remote work option of 2 days per week from home.
