Senior Community Services
, is a leading nonprofit organization with a mission to empower people as they age. We Reimagine Aging by serving compassionately, connecting communities, and supporting independence.

The agency seeks a full-time Licensed Social Worker to join its dedicated Senior Outreach & Caregiver Services team in providing quality services in Hennepin County. In addition to providing care coordination and supportive counseling for older adults, the program offers caregiver support groups, coaching/counseling, education sessions, and web-based tools for the caregiving family, friends, and neighbors of older adults.

Caregiving Support-Direct Service
  • Compassion, empathy and enjoyment working with older adults and their families.

  • Provide in-person and virtual services to participants.

  • Maintain an active caseload while supporting the coverage and management of client needs across the team during vacations and extended leaves.

  • Provide direct support, aging education, and advocacy to individuals and families facing day-to-day concerns as well as a dementia diagnosis, chronic illness, caregiving challenges, or other psychosocial concerns.

  • Conduct assessments to identify client and family needs, strengths, and barriers to care.

  • Develop individualized care plans and connect clients with appropriate community resources and services and fellow SCS programs.

  • Maintain accurate and timely documentation of client interactions, assessments, and service plans.

  • Comply with funder and agency documentation, reporting, invoicing, and audits.

  • Collaborate with SCS Development and Communications staff to promote programs through various channels, e.g., social media, newsletters, and public events.

Community Outreach
  • Ability to independently plan, prioritize, and execute territory development strategies while fostering relationships with healthcare providers, community organizations, and referral partners.

  • Conduct outreach and educational presentations to increase awareness for Senior Co mmunity Services programs and support groups.

  • Exhibit at community events, health fairs and outreach events for promoting SCS programs and community involvement.

  • Host office hours at various senior centers for drop-in assistance and SCS program promotion and visibility.

Su pport Group Facilitat ion
  • Knowledge of the physical, emotional, and psychosocial needs of individuals living with dementia and significant illness, as well as their caregivers.

  • Plan, organize, and facilitate support group meetings in a safe, inclusive, and supportive environment.

  • Provide emotional support, guidance, and appropriate resources to caregivers and care receiver.

  • Develop meeting agendas, educational materials, and activities that support group goals and participant well-being.

  • Understanding of grief, loss, and life transitions

  • Emotional R Resilience.

Additional Responsibilities
  • Work with development, finance, and executive leadership to support fundraising and grant opportunities.

  • Other duties as assigned

  • Experience developing relationships with people from diverse backgrounds, such as differences related to culture, religion, financial resources, race, national origin, age, gender, gender identity and expression, sexual orientation, and abilities.
  • Certification for Caregiving Consultation (Will provide time and training to secure this certification).
  • Commitment to support older adults living healthy and mean ingful lives.
  • Background in dementia training and significant illness support.
  • Minimum of 4 years of experience working with older adults.
  • Strategic thinker with the ability to innovate, plan and build a territory.
  • Initiative and I ndependent working
  • Effective relationship-builder with staff, clients, volunteers, and partners.
  • Strong listening, communication, and problem-solving abilities.
  • Demonstrated ability to maintain professional boundaries.
  • Excellent written and verbal communication skills.
  • Sense of H umor and positive mindset
  • Master’s degree – Preferred
  • Minnesota Social Work Licensure or Case Management experience preferred
  • Intermediate knowledge Microsoft Office Suite
