Applicants must have successfully completed the New Jersey Division of Developmental Disabilities (DDD) two-day Support Coordination Training and be able to provide a certificate of completion.
This position offers flexible scheduling, opportunities for remote work following training,
and the potential to transition into a full-time hybrid role.
- Develop and monitor individualized service plans for individuals with developmental disabilities.
- Assist individuals and families in accessing community resources and services.
- Maintain accurate and timely documentation in accordance with DDD requirements.
- Conduct required meetings, assessments, and follow-up activities.
- Advocate for individuals to support their goals, preferences, and quality of life.
- Collaborate with providers, families, and community partners to ensure effective service delivery.
- Bachelor's Degree in any field.
- Successful completion of the NJ DDD two-day Support Coordination Training and ability to provide certificate of completion.
- Minimum of one (1) year of experience working with individuals with developmental disabilities.
- Valid driver's license and reliable transportation.
- Strong organizational, communication, and computer skills.
- Part-Time (up to 30 hours per week)
