Med Zed seeks a Housing Navigator II to perform in-field housing placement and tenancy support for members with high acuity. You will assess needs, create service plans, connect to resources, and advocate with landlords, including home visits and documentation in our CRM.
This non-clinical field role requires at least 4 days/week in the field, with a hybrid/remote location and travel to meet members at their homes. Strong outreach and organizational skills are essential.
