Helen Ross McNabb Center seeks a Healthy Families Family Support Specialist to provide home-based services to at-risk families in Morgan County. In this role, you will conduct family assessments, create individualized support plans, and provide parenting education and child development guidance. You will link families to community resources, coordinate care with internal and external providers, and maintain accurate documentation. Working in a mission-driven, collaborative environment, you will support child safety, strengthen family functioning, and promote long-term well-being across East Tennessee.
Responsibilities
- Provide regular home visits to enrolled families in Morgan County to promote child health, safety, and development.
- Conduct family assessments and develop individualized family support or service plans.
- Educate caregivers on parenting skills, child development, and safe home environments.
- Connect families with community resources such as healthcare, housing, education, and financial assistance.
- Monitor family progress, document services, and maintain accurate, timely case records.
- Collaborate with internal clinical staff and external partners to coordinate care and referrals.
- Identify and respond to risk factors such as abuse, neglect, or domestic violence following agency protocols.
- Participate in team meetings, trainings, and supervision to ensure quality, evidence-informed services.
- Advocate for client needs within schools, healthcare providers, and community agencies.
