Healthy Families Family Support Specialist

Helen Ross McNabb Center seeks a Healthy Families Family Support Specialist to provide home-based services to at-risk families across East Tennessee. In this role, you will build trusting relationships, conduct regular home visits, and assess family strengths and needs. You will offer parenting education, developmental guidance, and emotional support, while monitoring child safety and growth. The specialist links families to community resources, collaborates with clinical teams, maintains clear documentation, and participates in supervision and training, helping promote hope, resilience, and healthy family functioning.

Responsibilities

• Conduct regular home visits with families enrolled in the Healthy Families program
• Assess family strengths, risks, and needs using standardized tools
• Develop and update family service plans to promote child safety and healthy development
• Provide parenting education, developmental guidance, and emotional support
• Connect families to community resources such as healthcare, housing, and childcare
• Collaborate with clinical and community partners to coordinate care
• Maintain accurate, timely documentation in electronic health records
• Monitor child development and safety, following mandated reporting requirements
• Participate in supervision, team meetings, and ongoing training
• Support program outcomes and quality improvement initiatives
