Job Summary
Make a meaningful impact in your community-join Acentra Health as a Clinical Evaluator (PASRR) in Omaha, Nebraska. In this hybrid role, you will conduct in‑person preadmission evaluations and assessments throughout Omaha and its surrounding areas.
As a Clinical Evaluator, you will conduct face‑to‑face PASRR Level II assessments to determine appropriate placements and next levels of care for individuals with complex needs. Your primary focus will be completing clinical evaluations for individuals with Serious Mental Illness (SMI), Intellectual Disabilities (), or Related Conditions (RC) across nursing homes, hospitals, and community‑based settings. These evaluations are crucial in determining the next level of care, improving patient well‑being, and supporting better health outcomes.
This PRN hybrid role offers flexibility and purpose, combining remote work from your home office with local travel as needed. Travel expenses—including mileage and parking—are covered and reimbursed by Acentra Health. We will provide all necessary technology equipment for your home office, including a laptop for use in the field.
Job Responsibilities- Responsible for conducting PASRR (Preadmission Screening and Resident Review) assessments and evaluations, and for developing informed level‑of‑care recommendations across diverse settings—including nursing homes, hospitals, and community environments. This includes determining the appropriate next level of care for individuals who may require long‑term care placement, while effectively assessing each individual's unique needs.
- Adhere to the state's best standards of mental health practices, intellectual and developmental disabilities, geriatrics, and other conditions requiring nursing care services and supports.
- Ensure compliance with contractual and regulatory standards while developing level of care recommendations.
- Utilize independent judgment, deep clinical expertise, exceptional communication skills, and problem‑solving abilities to navigate complex situations, ensuring the best outcomes for members, providers, and clients.
- Foster effective collaboration with a diverse range of healthcare professionals, patients, families, and support networks while monitoring personal metrics to ensure compliance with turnaround time requirements.
- Stay informed on current literature, regulations, and research pertaining to long‑term care, mental illness, medical conditions, and geriatrics.
- Leverage technology by utilizing the state's electronic system and our proprietary platforms to efficiently manage and track all assessment activities.
- Execute tasks with strong attention to detail, ensuring adherence to established workflows to maintain high standards of quality and operational efficiency.
- Must possess an active, unrestricted Nebraska license in one of the following:
Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), Licensed Marriage and Family Therapist (LMFT), Licensed Independent Clinical Social Worker (LICSW), Licensed Clinical Psychologist (LCP), Registered Nurse (RN), or another qualified Licensed Mental Health Professional (LMHP). - Graduation from an accredited college with a relevant degree program and minimum of 2 years of experience assessing individuals with mental health or psychiatric disorders, including complex differential diagnoses related to substance use, medical conditions, cognitive impairments, and dementia.
- Basic knowledge of Preadmission Screening and Resident Review (PASRR) requirements, including applicable federal regulations and state‑specific rules, is a plus.
- Skilled in assessing and analyzing psychological, emotional, behavioral, and physical functioning, with basic knowledge of standards and best practices for determining Level of Care within mental health, geriatrics, and conditions commonly requiring long‑term care placement.
- Strong verbal, written, and electronic communication skills with the ability to engage effectively with individuals and groups.
