Moms In Motion is seeking dedicated Service Facilitators to connect with families across Virginia’s Medicaid Waiver programs (CCC+, CL & FIS, EPSDT). You’ll help families live safe, healthy, and independent lives by guiding them through complex processes.
As a Service Facilitator, you’ll visit clients locally, coordinate plans of care, and troubleshoot authorizations and timesheets while working remotely from your laptop.
