Social Worker MSW

Gentiva is seeking a compassionate Social Worker (MSW) to provide home-based psychosocial support to patients and families. In this role, you will complete psychosocial assessments, develop care plans, and coordinate services that promote dignity, comfort, and quality of life. You will collaborate closely with an interdisciplinary team, connect patients with community resources, support advance care planning, and provide emotional counseling and crisis intervention.

Responsibilities

• Conduct comprehensive psychosocial assessments for home-based patients and families
• Develop and implement individualized care plans to address social, emotional, and resource needs
• Provide counseling, crisis intervention, and emotional support to patients and caregivers
• Educate and support families with advance care planning and end-of-life decisions, as appropriate
• Coordinate services with the interdisciplinary team, including nurses, physicians, and therapists
• Connect patients with community resources, financial assistance, and support services
• Advocate for patient rights, safety, and dignity in all care settings
• Maintain accurate, timely documentation in the electronic health record (EHR)
• Participate in case conferences, team meetings, and quality improvement initiatives
• Adhere to ethical standards, regulatory requirements, and agency policies
