Socket.dev in Oklahoma is seeking a Case Manager to join a two-year, multi-organization effort delivering in-person, housing-based case management to stabilize and house at least 40 households during the funding period.
You will coordinate with Outreach, Landlord Engagement, and Closure Maintenance teams, perform HMIS enrollments, assist with housing applications, move-ins, and provide weekly home-based support including intake, planning, budgeting, and connections to health services.
