This role focuses on protecting adults aged 60 and older who may be at risk of abuse, neglect, exploitation, or unsafe living conditions. We will rely on you to identify and coordinate services needed support consumers in meeting their social, economic, emotional, health, and physical safety needs.
As an Aging Care Manager 2, you will perform the following duties:
- Caseload Management:
Maintain a caseload of vulnerable consumers, visit homes, and follow up with each consumer and their formal and informal supports as needed - Consumer Support:
Advocate for consumers to help connect each person with the services they need to be safe, stable, and connected to their communities; testify at court proceedings - Reports of Need Investigation:
Interview consumers, family members, and other contacts; review relevant records; and evaluate information and apply regulations to determine appropriate next steps - Protective Service Intervention:
File emergency petitions, provide referrals for community based services and assessments, and pursue other remedies as warranted by investigative findings - Professional
Collaboration:
Work with legal, medical, financial, and other appropriate professionals to reduce hazards, end danger, and stabilize consumers - Confidential Documentation:
Maintain case records including intakes, assessments, care plans, correspondence, referrals, and progress notes in accordance with agency policy and directives
- Full-time employment
- Work hours are 8:30 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch. May be required to be on a standby rotation for off-hours coverage.
- Telework:
You may have the opportunity to work from home (telework) part-time. This position combines office and fieldwork. After the initial training and probationary period, telework may be considered on a case-by-case basis. The ability to telework is subject to change at any time. - Six months as an Aging Care Manager 1 or a County Caseworker 1;or
- Successful completion of the County Social Casework Intern program;or
- Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field;or
- An equivalent combination of experience and training.
