Role Description

The Clinical Case Manager is a full-time, hybrid role based in the San Francisco Bay Area, with a blend of on-site and work-from-home responsibilities. This role provides comprehensive case management for clients engaged in HUD-VASH and vocational rehabilitation services, including intake assessments, treatment planning, and ongoing monitoring of progress. Day-to-day tasks include coordinating social and mental health services, advocating for client needs, facilitating referrals to community resources, and supporting clients in securing housing, benefits, and employment-related services.

The Clinical Case Manager maintains accurate documentation, prepares reports, and participates in interdisciplinary team meetings to ensure continuity and quality of care. The role may also involve collaboration with legal and administrative stakeholders and participation in expert evaluations as needed.

Qualifications
  • Demonstrated experience in Social Work and Social Services, with a focus on working with vulnerable or underserved populations.
  • Strong background in Case Management and Case Management Services, including assessment, care planning, coordination, and follow-up.
  • Knowledge of Mental Health systems and services, and experience supporting individuals with mental health or co-occurring challenges.
  • Familiarity with HUD-VASH, housing programs, and vocational rehabilitation services is highly beneficial.
  • Relevant licensure or certification (e.g., LCSW, MFT, LPCC) or equivalent clinical experience preferred.
  • Ability to work effectively in a hybrid environment, manage time independently, and collaborate with multidisciplinary teams.
  • Strong written and verbal communication skills, including clear documentation and professional correspondence.
  • Bachelor’s or Master’s degree in Social Work, Psychology, Counseling, or a related human services field preferred.
