Our Coordinators of Community Services (CCS) partner with individuals with intellectual and developmental disabilities and their families to understand what matters most to them, navigate services and resources, advocate for their needs, and support progress toward meaningful life goals.

This is rewarding, relationship-centered work that also requires independence, organization, accountability, travel, timely documentation, and consistent follow-through. We want candidates to understand both the purpose and the day-to-day realities of the position before joining our team.

WHAT THIS ROLE IS REALLY LIKE

This is a community-based position—not a traditional remote job.

Administrative work may be completed from a home-based workspace, but regular in-person community work is an essential part of the position.

You will routinely travel throughout your assigned service region.

Meetings may occur in individuals' homes, provider locations, workplaces, programs, and other community settings.

Travel varies according to caseload needs and may occasionally involve driving up to approximately two hours one way.

Applicants are strongly encouraged to apply for positions within or near your residence.

You must have reliable transportation and the ability to travel throughout your assigned region.

WHAT YOU WILL DO

As a CCS, you will develop relationships with individuals with intellectual and developmental disabilities and their families and coordinate services around their needs, preferences, choices, and goals.

Responsibilities include:

Build trusting, person-centered relationships with individuals and their families or support systems.

Learn what matters most to each person and help translate their goals into meaningful supports and services.

Coordinate and monitor services across providers, agencies, community resources, and other support systems.

Connect individuals and families with appropriate community, healthcare, employment, housing, educational, financial, and social resources.

Monitor whether services are meeting the individual's identified needs and goals, and advocate for the individual when barriers to services or supports arise.

Participate in planning meetings and collaborate with individuals, families, providers, State agencies, and community partners.

Maintain accurate, timely, and person-centered documentation of case management activities in accordance with organizational and regulatory requirements.

Manage an assigned caseload while meeting quality, timeliness, productivity, and compliance expectations.

Participate in supervision, training, quality review, and professional development.

Maintain confidentiality and protect sensitive information.

Perform other duties reasonably related to the position.

Documentation and Productivity Matter

Case management does not end when the meeting or phone call is over. CCS employees are responsible for documenting the services they provide accurately and within required timelines. Documentation must clearly reflect the activity performed, the person's needs or goals, the connection between activity and the person's goals, and appropriate next steps. This position also includes defined productivity expectations.
