Position: Hospital to Home Discharge Worker

We have an incredible new opportunity for a Hospital to Home Advisor to join Landau. This role offers more than a wage – it’s about the deep reward of guiding young people who’ve faced challenging starts in life toward brighter futures. As a Hospital to Home Advisor, you will be responsible for providing person-centred support to older adults (65+) transitioning from inpatient mental health services to home, promoting independence, reducing isolation and preventing readmission.

Support starts at point of referrals and is time-limited (up to 4 weeks) once discharged. The role works as part of a multidisciplinary team (MDT), supporting delivery of discharge plans and connecting individuals to community-based support.

Key responsibilities will include:

Service Delivery

Engage service users on wards prior to discharge to build rapport and confidence

Deliver structured, short-term support in the community (home visits, clinics, telephone)

Support implementation of MDT discharge plans

Provide practical and emotional support to promote safe transition home

Facilitate access to community services, VCSE organisations and local assets

Reduce social isolation through community integration and engagement

Partnership & MDT Working

Attend ward-based MDT meetings and contribute to discharge planning

Work collaboratively with MPFT clinical teams, social care and VCSE partners

Build strong relationships with VCSE and community partners, maintaining up-to-date knowledge of local support services and identifying and feeding back gaps in provision to strengthen pathways and improve outcomes

Advocate on behalf of service users to improve access and outcomes

Maintain effective communication with carers and families (where appropriate)

Promote the service – attend forums and MPFT/ external team meetings / mental health

Caseload Management

Manage a caseload of up to 20 service users

Deliver time-limited interventions (maximum 4 weeks per individual)

Maintain accurate, timely records on RiO

Monitor progress and escalated risks appropriately

Outcomes & Impact

Support improved independence and confidence

Reduce risk of relapse and readmission

Improve engagement with community services

Contribute to positive service user and carer experience

Safeguarding & Risk

Identify and respond to safeguarding concerns

Work within MPFT and organisational safeguarding procedures

Escalate concerns via MDT and appropriate safeguarding routes

Compliance & Quality

Complete all mandatory training (e.g. safeguarding, dementia, suicide awareness)

Maintain GDPR and data governance standards / Health & Safety

Contribute to service monitoring, reporting and outcomes tracking
