The Social Worker
- Population Health plays a key role in supporting patients across the continuum of care by addressing social determinants of health, connecting individuals with community resources, and collaborating with interdisciplinary care teams to improve health outcomes. This position works closely with patients, families, providers, and community partners to identify barriers to care and develop individualized plans that promote wellness, independence, and access to needed services.

This hybrid role offers a combination of remote work and on-site collaboration while supporting Mercy's mission of compassionate, patient-centered care.

• Assess patients' psychosocial, environmental, financial, and support needs.
• Develop and implement individualized care plans that address barriers to health and wellness.
• Connect patients and families with community resources, financial assistance programs, behavioral health services, transportation resources, and other support systems.
• Collaborate with physicians, nurses, care managers, and other members of the interdisciplinary team to coordinate care and improve patient outcomes.
• Provide crisis intervention, advocacy, and ongoing support for patients with complex medical and social needs.
• Document assessments, interventions, and outcomes in accordance with organizational and regulatory requirements.
• Monitor patient progress and adjust care plans as needed to support successful health outcomes and reduce avoidable utilization.
• Promote patient engagement, self-management, and preventive care initiatives.

• Bachelor's degree in Social Work required;
  Master's degree in Social Work preferred.
• Current social work licensure, if required by state regulations.
• Previous experience in healthcare, case management, care coordination, population health, community health, or social services preferred.
• Strong knowledge of community resources and social service programs.
• Excellent communication, organizational, and relationship-building skills.
• Ability to manage multiple priorities in a fast-paced, collaborative environment.

Our Mercy health system was founded by the Sisters of Mercy in 1986. But our heritage goes back more than 195 years. It began with an Irish woman named Catherine McAuley, who was determined to help women and children in Dublin experiencing poverty. Though Catherine had a modest upbringing, she received an unexpected inheritance that allowed her to fulfill that dream.

In 1827, she opened the first House of Mercy in Dublin, intending to teach skills to women and educate children. Many volunteers came to help. A few years later, Catherine founded the Sisters of Mercy, one of the first religious orders whose Sisters were free to walk among the poor and visit them in their homes. In 1843, the Sisters of Mercy came to the United States.

In 1856, they traveled to St. Louis. In the years to come, they would establish hospitals throughout the Midwest. Many of these would join together in 1986 as Mercy.
