Role:
Care Coordinator for a Birth to Three Program supporting children aged 0–3 with developmental delays or disabilities and their families.
Responsibilities:Conduct functional assessments, develop and implement family-centered service plans, coordinate services, support families through education and advocacy, facilitate team meetings, and maintain accurate documentation. Collaborate with internal teams and community providers, manage a flexible schedule, and meet productivity expectations.
Qualifications:Bachelor's degree in Social Work, Child Development, or related fields; one year of supervised experience preferred. Valid driver's license, reliable transportation, strong organizational and communication skills, and ability to work in diverse settings and respond to crises.
Work Environment &Skills:
Community-based role involving travel, physical activity, and home/community visits. Must be adaptable, organized, and capable of working in home, office, and community settings.
