Humana is seeking a licensed Master’s-level Social Worker to join our SNP program in a remote, work-from-home setting. You will conduct telephonic outreach, assess social determinants of health, coordinate care with plan benefits and community resources, and engage with interdisciplinary teams to support member goals.
You will document all interactions in the member record, ensure CMS compliance, and manage care plans, with emphasis on patient-centered outcomes and timely follow-up.
