Job Description

A Home-Based Case Worker provides and connects families and children with the necessary tools and resources to sustain their everyday needs. As a provider, it is essential to assess the needs of each client in order to obtain necessary goals, and to complete tasks. Home-Based Case Work also includes helping people improve on making/obtaining goals, child care, coping skills, life skills, hygiene, employment, housing, communication, enhancing resilience, healthy relationships, safety, conflict management, problem solving, advocacy, budgeting, parent education, and knowledge of community resources.

Mentorship and advocacy are very essential for this position to ensure the client is being properly supported and assisted. A Visit Facilitator is to protect the integrity of the visit and to provide a positive atmosphere where parents and children may interact in a safe structured environment in the home, if permitted, or in a safe public area in the community.

This visit should be supervised by the facilitator to make sure the parent and child are bonding, reconnecting/reestablish, and strengthening the parent-child relationship dynamic in an appropriate manner. Conversations during visits or in transport concerning addresses, foster placement, or the case are strictly prohibited..

Essential Duties and Responsibilities

  • ● Create a consistent schedule for parent and child to meet weekly.
  • ● Transport child(ren) to visiting parent in the community or in the home if permitted.
  • ● Understand the levels of Supervised Visitation.
  • ● Complete Supervision intake Assessment with visiting parent.
  • ● Ensure the Client Handbook is read, understood, and signed by the visiting parent.
  • ● Complete Protective Factors Survey every 90 days.
  • ● Always pick up the child first, and drop off the parent first.
  • ● Ensure there is no discussion about the case, address of homes or school, or foster placement.
  • ● Document pertinent info on each visit within 24 hours of the visit thoroughly and in detail.
  • ● Create a safety plan with visiting parent.
  • ● Monitor, document, report, and address any safety concerns in Case Management Pro.
  • ● Complete a monthly report by the 10th of every month.
  • ● Maintain confidentiality within CSS policies and procedures.
  • Assess the needs of the child or family in order to make short and long term goals to obtain
  • Provide and connect the child and or family with necessary community resources
  • Provide mentorship and advocacy to ensure the client is being supported accordingly
  • Document each session in Case Management Pro within 48 hours of meeting.
  • Document visits and monthly reports in detail via Case Management Pro.
  • Create a safety and treatment plan in Case Management Pro.
  • Ensure the Client Handbook is read, understood, and signed by the client.
  • Assist with improving caretaking and coping skills, mental/physical/emotional support, and tactics in order to improve family interaction and well-being.
  • Monitor, document, report, and address any safety concerns in Case Management Pro.
  • Document evidence based models used in each session in Case Management Pro.
  • Create safety and crisis response plans for record keeping.

Educational Requirements/Experience

Paraprofessional Level Direct Worker

  • ● High school diploma or GED
  • ● Bachelor’s or Master’s degree in a directly related human services field. The individual must possess one of the following:
