EAC Network in Central Islip, NY is seeking a dedicated full-time Case Manager for the Alternatives for Youth program. You will support families in crisis with intensive home-based case management and crisis intervention, using a strengths-based approach to promote stability.
Responsibilities include conducting CANS assessments, developing intervention plans, and coordinating with community agencies. Strong English/Spanish communication and a valid NY driver’s license are required.
