Location: Ligonier
Indiana Professional Management Group (IPMG) is seeking a Case Manager to serve Noble County, IN. This full-time, hybrid role combines remote work from home with local travel for in-person community meetings several times a week.
Applicants must reside within Noble County and be capable of travel across Noble and surrounding counties. You will educate individuals about Medicaid waivers, build individualized support plans, help them access needed services, and lead community-based team meetings
