Since 2021, we've built our reputation on compassionate, compliant, person-centered care, and our Client Care Managers are at the heart of it.
About the RoleAs a Client Care Manager, you'll own a caseload of clients receiving home-based services and serve as the primary link between clients, families, caregivers, and our office team. This is a field-based role: you'll be out in the community conducting home visits, supervising care delivery, and making sure every client's plan of care is followed and documented.
What You'll Do- Manage an assigned caseload of clients, including regular home visits and care plan reviews
- Supervise and support caregivers in the field, including coaching and performance feedback
- Coordinate with service coordinators, families, and Nebraska DHHS as needed
- Maintain accurate, timely documentation in our care management system
- Monitor compliance with Medicaid waiver requirements, client rights, and agency policies
- Participate in caregiver onboarding, orientation, and ongoing training
- Respond to client and caregiver concerns with urgency and professionalism
- Bachelor's degree in social work, human services, healthcare administration, nursing, or a related field required
- Experience in home care, case management, social services, or a related field (Medicaid waiver experience a strong plus)
- Strong organizational and documentation habits; compliance matters here
- Excellent communication skills with clients, families, and field staff
- Comfort working independently in a field-based role
- Valid driver's license, reliable transportation, and ability to travel within the service area
- Ability to pass required background checks; CPR/First Aid certification (or willingness to obtain)
