Monday-Friday - you'll get to be active in the community and clinic, meeting clients where they are. You may even get to work from home Fridays after training and review!

About

CPLC Centro De La Familia provides outpatient behavioral health services for families and individuals of all ages. The program offers counseling and therapy services to those living with serious mental illness, general mental health issues, and substance abuse issues. These issues can be related to depression, anxiety, relationships, loss, substance abuse, suicidal thoughts, and trauma from sexual, emotional and/or physical abuse.

Outpatient services offer flexible office hours and home-based services for children and their families, public transit accessibility, therapy in English and Spanish, and professional staff who utilize best practice models for treatment.

WHAT MAKES YOU A GREAT HIGH NEEDS CASE MANAGER?
  • You build trust with youth, families, and community partners, using a strengths-based, family-centered approach to engage clients, coordinate services, and support positive outcomes.
  • You are organized and proactive, able to manage multiple priorities, maintain accurate documentation, meet deadlines, and ensure compliance with program, agency, and regulatory requirements.
  • You are a problem-solver and advocate, identifying barriers to care, connecting clients to resources, coordinating with multidisciplinary teams, and helping youth and families navigate complex systems.
  • You are adaptable and community-focused, comfortable working in homes, schools, clinics, and community settings, traveling throughout the service area, and responding effectively to the changing needs of youth and families.
  • Your familiarity of spectrum of mental illnesses, DSM-IV-TR/DSM 5 codes and descriptions, psychotropic medications and crisis intervention.
HOW WILL YOU MAKE AN IMPACT?

As the High Needs Case Manager (HNCM) facilitates the strategic planning and Child and Family Team (CFT) process for assigned youth and families. You will engage families, stakeholders, formal/ informal supports and individuals involved in the CFT on behalf of the youth and family for the primary purpose of developing a strengths-based plan which meets the youth and family’s needs.

This position is established to serve as part of a team of support for youth and their families enrolled in the behavioral health system. You're work will support The Arizona Vision: “In collaboration with each child, family and others, Arizona will provide accessible behavioral health services designed to aid children to achieve success in school, live with their families, avoid delinquency and become stable and productive adults.

Services will be tailored to the child and family and provided in the most appropriate setting, in a timely fashion, and in accordance with best practices, while respecting the child and family’s unique cultural heritage.”

  • Conduct comprehensive assessments and develop individualized service plans with youth, families, and care teams to identify needs, build on strengths, and support meaningful functional outcomes.
  • Coordinate care across medical, behavioral health, community, and out-of-home providers, ensuring continuity of services, advocacy, discharge planning, and participation from all stakeholders involved in treatment.
  • Provide ongoing case management and client support through office, home, community, and telehealth visits, connecting clients to resources, addressing barriers, developing life skills, and promoting self-advocacy and independence.
